Please note that we sell wholesale only, and our Minimum Order is $50.00 ($100.00 for new accounts).
This form is a convenient quick-method form for our customers who would like to place an order AND who know their item numbers/quantities; we will email you to confirm receipt of your order. If you do not know your Account Number (it is the 6-digit number listed above your name on your recent invoice), then you may leave it blank. If you are a new customer, then leave it blank and we will assign you an account number separately.
For "payment," please specify if you want to pay with Credit Card (Visa/MC), COD, etc - do NOT type in any credit card number! We will phone or fax you for your credit card number.
For "shipping" please advise if you want us to ship your order via UPS Ground, UPS Air <Next Day, Second Day, Three Day>, USPS Priority Mail, etc.
